Like all medications, the Semaglutide/B12 injection can have side effects. Common ones include:

Nausea

Vomiting

Diarrhea

Abdominal pain

Decreased appetite

These side effects are often temporary and may lessen as your body adjusts to the medication. However, if they persist or worsen, you should contact your healthcare provider.

Inform your healthcare provider if you have a history of thyroid cancer or a type of thyroid disease called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Before starting semaglutide/B12 injections, inform your healthcare provider about any allergies or medical conditions you may have, especially if you have a history of pancreatitis, kidney disease, or liver disease.

Furthermore, while B12 is generally safe, in very high doses, it might interact with certain medications. Let your healthcare provider know about all medications and supplements you’re currently taking.

Lastly, semaglutide may affect your blood sugar levels, so be sure to monitor your blood sugar as directed and share the results with your doctor.

The purpose of this consent form is to provide written information regarding the risks, benefits and alternatives of treatment with semaglutide and Vitamin B12. This material serves as a supplement to the discussion you have with your Crawford Cosmetics provider. It’s important that you fully understand this material, so please read this information thoroughly. If you have any questions regarding the medications and process, ask your Crawford Cosmetics provider prior to signing this consent form.

TREATMENT GOALS AND DESCRIPTION OF PROCEDURE Semaglutide with Vitamin B12 is an injectable compounded medicine used for adults with obesity or overweight (excess weight) to help them lose weight and keep the weight off. The compound formulation is a combination of Semaglutide and Vitamin B12 which are targeted to treat excess weight and alleviate potential side effects from the Semaglutide such as nausea. Given the original trials and studies did not contain Semaglutide and Vitamin B12, outcomes and side effects may differ. Semaglutide should be used with a reduced-calorie meal plan and increased physical activity. The procedure involves a weekly subcutaneous injection and a monthly physical exam. This procedure may be considered medically unnecessary. It may or may not mitigate, alleviate, or cure the condition for which it has been prescribed. This therapy has been recommended to you in the belief that it is of potential benefit in these circumstances and its use will probably improve the condition for which you are under treatment and your overall health.

TREATMENT RESULTS Most patients are pleased with the results of Semaglutide and Vitamin B12. However, there is no guarantee that you will be completely satisfied. Since every human being is unique, we cannot guarantee any specific result from Semaglutide and Vitamin B12 treatment. Treatment will be discontinued after 12 weeks if you have not lost at least 5% of your initial body weight. In trials of Semaglutide at 2.4mg dosing (such as STEP 1 trial) the average weight loss 14.9% compared with 2.4% without Semaglutide. 86.4% of people lost at least 5% of their body weight. Like with many other forms of weight loss medications as well as gastric bypass, weight loss tends to plateau or level off after approximately 68 weeks for Semaglutide. After discontinuation of Semaglutide, weight gain can occur especially if a healthy lifestyle and diet are not maintained. Given the original trials and studies did not contain compounded Semaglutide and Vitamin B12, outcomes and side effects may differ.

AFTERCARE INSTRUCTIONS Following your procedure, we’ll provide you with verbal and/or written aftercare instructions. If at any time you suspect you’re having an unexpected side effect, adverse reaction or complication, contact us immediately, and obtain help in an appropriate medical setting (i.e. Crawford Cosmetics, your primary care provider, specialist, urgent care, or acute care facility). I will adhere to the aftercare instructions supplied to me during my visit.

To hold this appointment, we require a credit card on file, and partially refundable $75 deposit for appointments under 30 mins ($67.50 will be refunded), and $200 partially refundable deposit for appointments over 30 mins ($180 will be refunded). Shorter appointments with our estheticians require a smaller deposit and are partially refundable. Other appointment deposits many vary. In order to be respectful of your fellow clients, please notify your Crawford Cosmetics provider as soon as you know you will not be able to make your appointment.

Please note, if you are more then 10 minutes late for your scheduled appointment, your appointment will be automatically cancelled, you will lose your deposit, and will be charged the additional no show fee of $100.

If you have a Tuesday appointment, we require that you cancel the Thursday before by 5pm at the latest. Appointments are in high demand, and your advanced notice will allow another client access to that appointment time.

Semaglutide + B12

Semaglutide targets an area in the brain that regulates appetite and food intake. This slows your digestion and in return makes you feel fuller faster and curbs cravings! At Crawford Cosmetics we carry a compounded Semaglutide medication with B12 in it. The additive B12 helps prevent the most common side effects of GI issues.

The Semaglutide/B12 injection can be a beneficial treatment for those looking to lose weight. By reducing appetite, increasing metabolism and energy levels, it can help improve your weight loss efforts. However, remember that this medication should be used in conjunction with a healthy diet and regular physical activity for optimal results. As with any new medication, always consult your healthcare provider if you have any questions or concerns.

Tirzepitide

Tirzepatide works by slowing down how food moves from the stomach to the small intestine, which can help you feel full for longer. It should be used in combination with diet and exercise. It is the generic for Mounjaro and Zepbound.

In clinical trials, people who took tirzepatide, dieted, and exercised lost an average of 13–21% of their body weight, compared to about 3% for those who took a placebo. However, some studies have shown that people who stop taking tirzepatide can regain a significant amount of the weight they lost. That is why it is extremely important to learn healthy habits while on the drug and to maintain these habits after stopping the medication.

Tirzepatide mimics hormones produced in the gastrointestinal system, so side effects can include nausea, vomiting, diarrhea, or constipation. These side effects usually resolve over time, and few people drop out of studies due to them.

Tirzepatide

Tirzepatide, often known under the brand name Mounjaro or Zepbound, is a glucagon-like peptide-1 (GLP-1) receptor agonist. It has been approved for chronic weight management in adults.

Tirzepatide works by mimicking the function of a natural hormone in your body called GLP-1. It helps to regulate your appetite, reducing feelings of hunger and helping you eat less.

The injection is usually administered once weekly. You should follow your healthcare provider’s specific instructions for the dosage and frequency of injections. The medication is typically injected under the skin (subcutaneously) in the stomach, thigh, or upper arm.

Make sure to rotate the injection site each time to reduce the risk of developing skin reactions. Never inject into a vein or muscle.

Tirzepatide Per Month 1-4     $500 / $450 CC

Tirzepatide Per Month 5-6     $750 / $700 CC

B12 Shot

B12 injections are typically used as a treatment for a certain type of anemia (pernicious anemia). In this type of anemia, people lack intrinsic factor in the stomach which is necessary for the absorption of the vitamin. Vegetarians (especially vegans) are also given shots of B12 since their diet is low in animal products, the primary source of B12. People with chronic fatigue or anemia require weekly to monthly infections of vitamin B12 usually because the oral form is not dependable. Vitamin B12 shots are most effective when taken at regular intervals (usually weekly or monthly). A regular schedule to receive the injections can be customized to each individual. The body’s ability to absorb vitamin B12 is reduced with increasing age. Older people are often detected to have a more vitamin B12 deficiency, even in cases where they do not suffer from pernicious anemia. Deficiently of vitamin B12 can lead to abnormal neurologic and psychiatric symptoms including ataxia (shaky movements and unsteady gait), muscle weakness, spasticity, incontinence, hypotension, vision problems, dementia, psychosis, and mood disturbances.

LIPO Shot

If the conventional method of diet and exercise for losing weight isn’t working for you, then look no further. You may be in need of the essential vitamins and minerals that enable you to lose the fat and burn off the pounds. The body requires nutrition and vitamins to stimulate the right hormones to function correctly so your body will let go of the fat stores it’s been holding onto.

A lipotropic nutrient is what’s used in the body to promote, encourage, and support the removal of fat from the liver. It also aids in ensuring that excess fat gets burned off for fuel and energy instead of being stored away. This trapped bile and fat can also cause the metabolism to slow down, and it can even block fat metabolism completely. This has interrupted the natural fat burning processes and prevented the body from using and discarding fat as it should.

Your dose will depend on your body mass and other health conditions. Intramuscular injections allow for the slow release of ingredients directly into the bloodstream. Although oral products containing Lipotropic ingredients are available, the digestive system does not absorb them efficiently enough and this limits their efficacy.

A Lipotropic injection contains a solution of nutrients that are made up of one or more of the following fat burning compounds. Each of these components is important because they each have a key role in utilizing fat, as fat burners, distributing energy, and removing toxins from the body. The overall effect of lipotropic injections is enhanced weight loss, particularly when they are combined with a healthy diet and exercise program.

Ingredient Breakdown
  • B6 – Promotes red blood cell production and converts food into energy.

  • Methionine – Helps break down sugars & carbohydrates and convert them into energy.

  • Inositol – Converts food into energy.

  • Choline – Cuts muscle recovery time, helps convert fat into energy.

  • B1 Thiamine – Improves your immune system, and helps convert fat and carbs into energy.

  • B2 Riboflavin – Increases metabolism and supports your immune system.

  • Carnitine – Improves fat metabolism and energy, reduces recovery times, and promotes muscle-building potential.

Improves Heart Health

Thanks to the various vitamins, minerals, and nutrients that make up the lipotropic family, you could see a decrease in cholesterol levels. In addition to incorporating a healthy diet and exercise, your risk for heart disease in general goes down.

Feel Happier

When lipotropics are activating fat loss, hormones are released, improving mood. This stimulates an increase in energy & it can reduce depression. It also helps to improve concentration & mental clarity. Inositol, is involved in the process which shows more efficient nervous system signal transmittal. This benefit is associated with preventing memory loss.

Liver Love

This is one of the biggest benefits that lipotropics provides, taking care of your liver. They help to keep it clean by aiding it in the filtering process, like a detox. It also protects it from bile and toxins and from decreasing effectiveness and tissue damage.

Hormone Balance Restoration

If your hormones are out of whack, they can be evened out on the scales by lipotropic injections.

Gut & Appetite Health

If you’re dieting to enhance weight loss, then you’ll be pleased to know that lipotropics can also act as an appetite suppressant. Since these nutrients are very involved in metabolic processes, you can also expect that your entire digestive system will be more efficient too.

Nail & Skin Health

With lipotropic injections, you might notice stronger nails and shinier hair due to a boost in essential vitamins and minerals.

Immunity Boosting

Inositol, can aid the body in eradicating sickness and disease. Its been known to help relieve or prevent various conditions such as insomnia, depression, ADHD, panic attacks, diabetic nerve pain, autism, cancer, and psoriasis.

Arthritis Issues

Thanks to the lipotropic Methionine, it encourages healthy growth, formation, and protection of cartilage of the joints.

More Methionine

In addition to its arthritis-scaring talent and its fat-burning effects, it’s also used to treat copper poisoning, asthma, alcoholism, Parkinson’s Disease, allergies, and nuclear radiation poisoning.

Research shows that receiving lipotropic injections on an empty stomach can maximize the results of the treatment. This will ensure that the injection goes straight into the bloodstream. Refrain from drinking anything 30 minutes before and eating anything an hour before the injection. This will keep the fat levels low in the blood while the lipotropic injections get into the system.

The patient must receive the shots at the same time during each appointment. If the injections started in the afternoon, then the rest should be at the same hour of the day. The scheduling must be set on the first appointment. Consistency will yield the ideal results.

Although side effects to the injections are rare, and people generally tolerate them well, there may be a few who can experience mild adverse effects. Lipotropic injections side effects that can occur with the simple use of application can be redness, swelling, and infection at the injection site. These symptoms should resolve fairly quickly so if they persist, you may have contracted an infection and should consult your doctor.
