DO I NEED TO USE TOPICAL ANESTHESIA? WHAT ABOUT GELS?

No. Anesthetics and cooling gels are not needed with Neo Elite, because MicroPulse-1064 makes it uniquely pain-free. However, some buildup of heat will be experienced and slight pinching may be felt while using the 2mm spot size. INFORMED CONSENT FOR AEROLASE LASER PROCEDURES

I have given Crawford Cosmetics permission to perform Aerolase laser procedure.

The Neo® laser is FDA approved for a variety of procedures including hair removal, vein treatment and wrinkle reduction. This form is designed to give you the information you need to make an informed choice of whether or not to undergo laser treatment. If you have any questions, please do not hesitate to ask. Although the laser treatment is effective in most cases, no guarantee can be made that a specific patient will benefit from the treatment.

The laser emits an intense beam of light that is absorbed in specific body tissues within the skin, and depending upon the type of procedure, several treatments may be required at intervals specified by the physician.

Some of the possible complications of laser treatment are:

1. Discomfort – The procedure is done so precisely that surrounding tissue is minimally affected; the patient may experience a mild sensation of pain in the treated areas. Some degree of skin flushing may occur, but it typically resolves within several hours.

2. Scarring – There is a small chance of scarring, including hypertrophic scars, or very rarely, keloid scars. Keloid scars are very heavy raised scar formations. To minimize chances of scarring, it is important that you follow all postoperative instructions carefully. It is important that any prior history of unfavorable healing be reported.

3. Pigmented changes – The treated area may heal with lighter or darker pigmentation. This occurs more often in darker pigmented skin and following exposure of the area to the sun. It is recommended that you protect yourself from any sun exposure for at least three months following treatment. Hyperpigmentation usually fades in three to six months. However, pigment change can be permanent.

4. HSV Reactivation – The patient agrees to notify the physician if he/she has any history of Herpes viral infections, as the laser procedure may cause it to reactivate.

5. Lack of Treatment Response – There is a possibility that the targeted hairs, veins or other treated areas will not respond to the treatment. This is often a function of the specific body chemistry of the patient, including relative pigmentation and light absorption characteristics of the patient’s various body tissues.

6. Eye Exposure – There is also the risk of harmful eye exposure to laser surgery. Safeguards should be provided by the laser practitioner. It is important that you keep your eyes closed and have protective eye wear at all times during the laser treatment.

7. Photographs – I consent to be photographed before, during, and after the treatment and that these photographs shall be the property of the above doctor and may be published in scientific journals or for scientific or marketing reasons.

Appointment Cancellation

Our policy enforces a late cancellation fee for cancellations made with less than a 72-hour notice. It is important to understand that you will forfeit your deposit for any appointment cancelled inside the 72-hour cancellation window. If you no call/no show, in addition to forfeiting your deposit, you will be charged a fee of $100. We take No call/No shows very seriously. We make all efforts to avoid these fees for you by sending multiple texts and email reminders that allow ample time to cancel or reschedule your appt with us.

When you book your appointment, you are holding a space on our calendar that is no longer available to our other clients. To hold this appointment, we require a credit card on file, and partially refundable $75 deposit for appointments under 30 mins ($67.50 will be refunded), and $200 partially refundable deposit for appointments over 30 mins ($180 will be refunded). Shorter appointments with our estheticians require a smaller deposit and are partially refundable. Other appointment deposits many vary. The deposit will be applied toward the cost of your treatment and is partially refundable if you do not receive treatment at the time of your appointment. In order to be respectful of your fellow clients, please notify your Crawford Cosmetics provider as soon as you know you will not be able to make your appointment.

Please note, if you are more then 10 minutes late for your scheduled appointment, your appointment will be automatically cancelled, you will lose your deposit, and will be charged the additional no show fee of $100.

If cancellation is necessary, we require that you call, text, or cancel online at least 72 hours in advance. Appointments are in high demand, and your advanced notice will allow another client access to that appointment time. As we are closed over the weekend, if you have a Monday appointment, we require that you cancel the Wednesday before by 5pm. If you have a Tuesday appointment, we require that you cancel the Thursday before by 5pm at the latest. How to Cancel Your Appointment

If you need to cancel your appointment and are enrolled in text message reminders, you can reply “no” to the reminder text that you will receive 4 days prior to the appointment. Late Cancellations vs. No-Shows

Some patients will not respond as well as others, and one factor is hair color. Some hairs don’t contain enough melanin to absorb the necessary energy to destroy or damage a follicle, and this is often the case with a light-colored hair (i.e., blond, gray, red). Another common factor to strong hair regrowth is hormonal imbalance, including PCOS (Polycystic Ovarian Syndrome).

Patients who are sunburned should wait until the burn subsides. Herpes Simplex Labialis (fever blisters) can be activated by laser treatment, especially on the upper lip, and patients with a tendency for fever blistering can be started on antiviral medication before treatment.

Setting expectations:

Patients need to be informed that a very good result with LHR is when approx. 15% clearance is achieved in each treatment session, and that an excellent final outcome of clearance of 80% or more after 6 treatments is exceptional. So, they should have realistic expectations of the results from each session and the final outcome, and in the end they should never expect 100% clearance. Also, some patients will respond very little, or not at all, even if all the settings and technique are properly applied; this is most likely due to hormonal conditions or other medical issues. The clinician should consider a checkup after the second treatment to ensure, with photographic verification, that hair clearance is progressing at the same or similar rate as described herein.

Does the patient need to shave before the procedure?

The patient should shave 2 or 3 days before the procedure, leaving small 2 to 3mm stubs of hair. This is critically important for several reasons:

1. visible stubs ensures the hairs haven’t been waxed or plucked, so the shaft and follicle are intact and therefore efficacy is not compromised;

2. seeing the stubs allows you to establish the appropriate fluence for the patient, which should be set at a level that causes most of the stubs to visibly singe;

3. seeing the stubs allows you to target the areas with actual follicles, rather than wasting time delivering pulses on the skin in between follicles; and

Can it be used on dark skin?
